COPD Action Plan

The COPDX Action Plan should be completed by the clinician and patient together. It guides the patient in recognising when their condition changes and what action they should take.
The COPDX Action Plan is available in different formats:
  1. Pads of 10 Action Plans which can be ordered from the Lung Foundation Order Form 
  2.  A pdf version COPD Action Plan  – this can be downloaded, printed and filled out in the practice
  3. A Rich Text Format version COPD Action Plan   – this format can be uploaded into Medical Director downloading templates into md
