Lung Foundation

All entrants must read the terms and conditions below and agree to abide by those terms and conditions by signing their entry form or checking the appropriate box on the electronic entry form.

• ‘Event’ means The Australian Lung Foundation Corporate Fun Run / Walk to take place on November 27 2008 which incorporates a 5km individual and corporate event.

• This form is not transferable – it is valid for the person(s) whose name and address appears on the individual or corporate Entry Forms for the Event stated.

• I/We acknowledge that it is a condition of participating in this Event that I/we do so at my/our own risk.

• I/we accept all risks and release the Event organizer, its agents, affiliates, employees, members, sponsors, promoters, volunteers, all instrumentalities, cities, Commonwealth or State councils where the event is held, and any person or body directly or indirectly associated with the Event, from all claims, demands and proceedings arising out of or connected with my/our participation in this Event. This release continues forever and binds my/our heirs, successors, executors, personal representatives and assigns.

• I/We acknowledge that participating in this Event may involve a real risk of serious injury or even death from various causes including over exertion, dehydration and accidents with other participants, spectators or Event course users.

• I/We recognize the difficulties of this Event and attest that I am physically fit to compete safely in this Event and that I have not been advised otherwise by a qualified medical practitioner.

• I/We have read Event conditions of entry and understand the physical nature of the Event. I/we have trained and prepared for this Event and I/we are not aware of any medical condition or impairment that will be detrimental to my health if I/we participate in this event. In the event that I/we become aware of any medical condition or impairment or am otherwise sick or injured prior to the Event, I/we will withdraw from the Event.

• I/We hereby acknowledge that I/we have sole responsibility for my/our personal possessions and athletic equipment during the Event and related activities.

• I/We hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident and/or illness during the Event.

• I/We understand that dogs are not permitted to accompany participants in the Event.

• I/We understand that my/our Race Number will be included in the event pack which will be given to be at the registration desk if I/we have registered prior to 8pm on Monday, November 24 2008.

• I/We understand that my/our entry form and payment is valid only for the Event stated.

• I/We understand that safety precautions undertaken by organizers are a service to me and others but not a guarantee of safety.

• I/We agree to abide by all race rules and directions issued by the Event organizers.

• I/We understand that if the Event is cancelled due to flood, cyclone, torrential rain, fire or other ‘acts of God’ conditions, the entry fee will not be refunded.

• I/We certify that I/we are over 18 years of age.

• I/We understand that if I/we do not provide the information requested, I/we may not be eligible to enter in the Event.

• I/We have read, understood and agree to abide by the terms and conditions for the Event with signing of the entry form or checking the apprpriate box on the online entry form.
